Bone-patellar tendon-bone allograft reconstruction for peri-patellar tendon sarcomas: case series.

چکیده
INTRODUCTION Reconstruction after wide resection for a sarcoma involving the knee extensor mechanism is challenging even if the tumor is small. CASE DESCRIPTION We report on four consecutive peri-patellar tendon sarcomas treated similarly at a single institution. Histological diagnoses were synovial sarcoma (two cases), clear cell sarcoma and extraskeletal Ewing's sarcoma (one case each). Follow-up periods after surgery were 18-67 months. All cases underwent pre-operative radiotherapy and subsequent surgery. After preoperative radiotherapy and wide resection including the patellar tendon, bone-patellar tendon-bone allograft was fixed to the residual patella and tibial tuberosity with screws and a cable wire. Soft tissue and skin defect over allograft was covered by free antero-lateral thigh flap. Post-operatively, the operated knee was splinted straight for at least 6 weeks, and then range-of-motion exercise was gradually introduced. Except for one case with a proximal tibial stress fracture 5 months post-operatively, no complication was observed. Both bone-bone junctions between allograft and residual bones were united within 1 year after surgery. At the latest clinical follow-up, all the patients had satisfactory functions with Musculoskeletal Tumor Society score of 28-30 out of 30 points and virtually full range of motion. DISCUSSION AND EVALUATION This case series is the first to report bone-patellar tendon-bone allograft for reconstruction after tumor resection with joint preservation and with satisfactory clinical outcomes. CONCLUSIONS Bone-patellar tendon-bone allograft reconstruction with vascularized flap reconstruction is a viable option for peri-patella tendon sarcomas.

Simultaneous bilateral rupture of patellar tendons in diabetic hemodialysis patient: A case report
Background: Bilateral rupture of the patellar tendon is a very rare injury, which takes place in relation to chronic systemic diseases. These injuries are known causes. Some of these causes are particular in patellar tendon rupture and another are in quadriceps tendon rupture. Case presentation: 70-year-old diabetic man with simultaneous bilateral patellar tendon disruption of proximal inser...
متن کاملIs there any Correlation Between Patient Height and Patellar Tendon Length?
Background: A potential specific problem of patellar tendon graft in ACL reconstruction is the possibility of graft-tunnel mismatch which could be more problematic with anatomic ACL reconstruction where the femoral tunnel is placed low on the lateral wall of the lateral femoral condyle. The occasional occurrence of this mismatch raises the question that whether a correlation exists between pat...
متن کاملThe modified bone-patellar tendon-bone allograft in single-bundle anterior cruciate ligament reconstruction.
Bone-patellar tendon-bone graft has been an attractive option for single-bundle anterior cruciate ligament reconstruction in clinical practice. However, the graft-tunnel mismatch in the proximal part of the tibial tunnel and the ultimate strength after postoperative ligamentization process have been potential problems for the traditional 10-mm wide graft. We modified the traditional bone-patell...
متن کاملAnterior Cruciate Ligament Reconstruction with or without Bracing
Background: Rupture of anterior cruciate ligament (ACL) would cause instability and disability and if left untreated. Reconstruction is performed by different techniques including patellar bone graft, as the strongest device. Application of brace after ACL reconstructions has revealed doubtful results. Methods: One hundred ACL ruptured patients randomized into two groups were reconstructed with...
متن کاملLate reconstruction of the patellar tendon in rheumatoid arthritis using bone-patellar tendon-bone allograft
INTRODUCTION chronic patellar tendon rupture is rare and its incidence and prevalence are unknown. Furthermore, the data about late reconstruction of the patellar tendon in rheumatoid arthritis is limited. Such condition, if not managed properly, can substantially affect the patient's activities of daily living. CASE PRESENTATION We report a case of chronic patellar tendon rupture in a 49 yea...
